Uveitis is a condition that affects the middle layer of the eye, which is known as the uvea. The uvea comprises the iris, ciliary body, and choroid. Any of these parts can become inflamed, causing Uveitis, which can affect one or both eyes and can occur in people of any age. Uveitis requires prompt medical attention because it can be severe and, if left untreated, can lead to complications such as Uveitis, cataracts, retinal detachment, or permanent vision loss.

Various factors can cause Uveitis. These may include:

  • Autoimmune disorders: Uveitis can occur due to the immune system mistakenly attacking the eye's tissues in conditions like rheumatoid arthritis or lupus.
  • Infections: Certain infections, including viral, bacterial, and parasitic, can lead to Uveitis. Examples include herpes simplex, tuberculosis, and toxoplasmosis.
  • Eye injuries or trauma: Uveitis can develop following direct injury or trauma to the eye, causing inflammation in response to the damage.
  • Inflammatory conditions: Uveitis can be associated with other inflammatory diseases like sarcoidosis or Behçet's disease, affecting multiple organs, including the eyes.
  • Idiopathic: In some cases, the specific cause of Uveitis remains unknown and is referred to as Idiopathic Uveitis.

  • Slit-Lamp Examination: - A slit-lamp is a specialized microscope that allows the doctor to examine the structures of your eye in detail. By using a bright light and magnification, the doctor can observe any signs of inflammation, such as redness, swelling, or abnormalities in the front part of the eye.

  • Dilated Eye Examination: - The doctor may administer eye drops to dilate your pupils, which allows for a more thorough examination of the back of your eye, including the retina and the optic nerve. This helps assess inflammation in the posterior part of the eye, which is common in Uveitis.

  • Ocular Pressure Measurement: - High intraocular pressure (pressure inside the eye) can indicate certain types of Uveitis, such as anterior Uveitis. The doctor may use a tonometer to measure the pressure in your eyes.

  • Laboratory Tests: - Sometimes, blood tests or other laboratory investigations may be ordered to look for specific infections, autoimmune markers, or underlying systemic conditions associated with Uveitis.

Procedure:

  • Anterior Uveitis: - Eye drops containing steroids help reduce inflammation in the front part of the eye. - Cycloplegic eye drops dilate the pupil and prevent muscle spasms in the iris and ciliary body.

  • Intermediate Uveitis, Posterior Uveitis, and Panuveitis:
    - Oral steroids may be necessary in addition to eye drops to control inflammation.
    - Injections, including periocular or intravitreal injections, may be recommended in some cases. These may contain sustained-release medications like steroid implants (e.g., Ozurdex) for longer-term treatment.
    - Immunosuppressive agents may be prescribed to suppress the immune system and reduce the need for long-term steroid use. Careful monitoring is required due to potential side effects.
    - In severe cases, vitrectomy surgery may be considered to manage significant inflammation or complications. This involves removing the gel-like vitreous substance from the eye’s middle to help control inflammation.
