Glaucoma is a condition characterised by damage to the optic nerve, resulting in vision loss and potential blindness. This condition is often associated with ageing but can also be caused by other factors such as eye injuries, certain medications, and certain medical conditions. Fortunately, early detection & timely intervention can help to halt the progression of glaucoma disease.

Various factors can cause glaucoma. These may include:

  • Ageing
  • Family history of glaucoma
  • Eye injuries
  • Certain medical conditions
  • Diabetes
  • Certain medications.

In early Glaucoma, there may be no symptoms at all in certain types of glaucoma following symptoms may occur:

  • Halos around lights
  • Eye pain
  • Headache
  • Nausea

As glaucoma progresses, the following symptoms may occur:

  • Tonometry: - This test measures the pressure inside your eye, known as intraocular pressure (IOP). Elevated IOP is a standard indicator of glaucoma. Tonometry can be done using various methods, such as the air puff test, applanation tonometry, or handheld tonometry devices.

  • Ophthalmoscopy: - During an ophthalmoscopy, your eye doctor will examine the optic nerve at the back of your eye. Changes in the appearance of the optic nerve, such as cupping or damage, can be indications of glaucoma.

  • Visual Field Test: - This test assesses your peripheral vision. It helps detect any loss of sight caused by glaucoma. You'll be asked to focus on a central point and indicate when you see flashing lights or other visual stimuli appear in your peripheral vision.

  • Gonioscopy: - This test involves using a unique lens to examine the drainage angle of your eye. It helps determine if the angle is open or closed, which is essential in classifying the type of glaucoma.

Procedure:

Advanced technology and laser procedures have improved glaucoma treatment by reducing intraocular pressure (IOP) to protect the optic nerve. Laser and Surgical options include:

YAG laser peripheral iridotomy:​ laser is used to create a small hole in the iris (the coloured part at the front of the eye) to help open the drainage angle and treat or prevent angle-closure glaucoma.

Trabeculoplasty: A laser treatment to enhance fluid outflow and lower IOP.

Trabeculectomy: Creating a new drainage channel to reduce IOP.

Minimally Invasive Glaucoma Surgery (MIGS): Microscopic devices create new pathways for fluid drainage.

These surgeries are typically performed under local anaesthesia and have short durations.

Post-Op Care:

Adhere to post-operative instructions and attend follow-up appointments for healing monitoring. Although rare, watch for risks like infection or bleeding. Avoid strenuous activities, protect your eyes from irritants, and use prescribed eye drops to aid healing.

Regular medication, such as eye drops or oral drugs, is often necessary for maintaining desired IOP and preserving vision. Follow the medication schedule as instructed by the Glaucoma specialist.

Open Angle Glaucoma
